FLEXIBLE SPENDING ACCOUNT (FSA) AND OTHER HIGH-DEDUCTIBLE, OR "CAFETERIA" PLAN CARDS CANNOT BE USED; PLEASE REQUEST "PAID" INVOICES FOR PURPOSES OF POSSIBLE* REIMBURSEMENT.

Please be sure to review SPC's No-Show Policy. Except in cases of actual emergency, if you fail to keep an appointment without 24 hours' advance notice, the card-on-file will be charged the full amount of your missed visit.
